Uniswap v4 hook with off-chain keeper-driven dynamic LP fees and a separate hook fee charged to a treasury on every swap. An operator configures per-pool min/max/default fee bounds; a pusher pushes the current fee within those bounds. If the keeper stops updating, the pool falls back to a default fee. The hook fee is minted to the treasury as ERC-6909 claims in afterSwap.
